The standard version of the Conditional Fields app is compatible with Zendesk Help Center forms. To ensure your changes appear correctly in the Help Center, you'll need to add a small code snippet to your theme.
How to deploy your configuration
Follow these steps to apply your conditional field logic to Help Center forms:

Save your configuration
In the Conditional Fields app, make sure you click the Save button after setting up your rules. -
Add the code snippet to your theme
Copy and paste the following code snippet into thenew_request.hbs
file of your Guide theme:
